Save time on your first visit
Your first visit is the start of your relationship with your doctor. Dr. Fraser will meet with you in his private consultation room before and after your first exam.
 
On your first visit, please bring:
  1. •Your current insurance card
  2. •Names and dosages of any medications you are taking
  3. •A list of your drug allergies and reactions
  4. •Medical history, including serious or chronic illnesses and previous surgeries
 
You may save time by bringing in completed forms.
Please print out the three PDF forms below and bring them with you on your first visit.
